We are open Monday to Friday, 8.00am – 5.00pm and closed on public holidays. An after-hours phone service is available. For medical emergencies, call 111.

How to enrol with us

Our GP Enrolment process

When enrolments are open, you and your family can register with us at West End Medical Centre. You can then use us as your regular and ongoing general practice provider or provider of First Level services. You can request an enrolment form at our reception desk or download it here.

Please note, you will not be able to make an appointment the same day you enrol. We require the transfer of your patient notes from your previous practice before making your first appointment. This may take several days. We check each new application to ensure you meet our enrolment criteria before we can complete your request.

To complete your GP enrolment, you need to provide:

  • Proof of eligibility, such as passport and proof of address – scanned/photocopied documents are acceptable
  • The name of your previous medical centre to transfer over your notes

Upon enrolment, your form will be reviewed to see if a face-to-face appointment is needed to update your medical records. If required, your first appointment at Westend Medical Centre may be 30 minutes long and cost $65. A nurse will update your medical history. Doctor consultations after enrolment cost $20.00 with a Community Services Card or $30.50 without, unless time limits are exceeded.

Why you need proof of eligibility

To complete your enrolment, you will need to provide originals of your eligibility proofs. We are unable to work on your enrolment with photocopies. In most cases, proof of eligibility will be one of the following documents:

  • Passport (NZ, or if from overseas with current visa)
  • Proof of address e.g. electric or telephone bill with your name and address on it
  • NZ certificate of identity
  • NZ birth certificate
  • Cook Islands, Niue or Tokelau birth certificate
  • NZ certificate of citizenship
  • Descent registration certificate
  • Social security benefit papers (except emergency benefit) and two forms of supporting identity information, one with a photo (unless a passport is provided)

How do I enrol at West End Medical Centre?

You can enrol to use West End Medical Centre as your regular GP or provider of First Level services by requesting an enrolment form at our reception desk. Once submitted, we check your application against our enrolment criteria before completing your request.

No. Your patient notes need to be transferred from your previous practice, which may take several days. Appointments can only be made once your enrolment and medical record transfer are complete.

  • Proof of address (e.g., electricity or telephone bill)
  • NZ birth certificate, certificate of citizenship, or descent registration certificate
  • Certificates from Cook Islands, Niue, or Tokelau

Photocopies are not accepted.

To enrol, you must reside in New Zealand and meet at least one of the following:

  • NZ citizen or permanent/resident visa holder
  • Australian citizen or permanent resident staying in NZ for at least 2 years
  • Work visa holder able to stay in NZ for at least 2 years
  • Interim visa holder eligible immediately before the interim visa started
  • Refugee, protected person, or in process of applying for protection
  • Under 18 under the care of a parent/legal guardian meeting the above criteria
  • NZ Aid Programme student or participant in certain educational schemes

Your enrolment form will be reviewed to determine if a face-to-face appointment is needed to update your medical records. If required, the first appointment may be 30 minutes and cost $65. Nurse-led updates are conducted, and doctor consultations after enrolment cost $20 with a Community Services Card or $30.50 without (unless time limits are exceeded).

Once enrolment is complete and your medical notes are transferred, you can access all services at West End Medical Centre, including GP and nurse appointments, prescriptions, and workshops.
